This alert discusses patient safety incidents of suicide and/or self harm of patient currently in hospitals. Four cases of suicide or injury are presented involving hanging, jumping from height, ingestion of cleaning substance and harm with a knife. Recommendations for preventing such adverse events are provided. In addition, the alert includes risk signs and potential factors of suicide, measurement tools for suicide risk, and criteria for accreditation of relevant medical institutions.
