This alert describes a patient suicide committed by jumping from a height away from the hospital. A lymphoma patient who had progressive disease for more than 6 years was admitted for neutropenic fever. The patient had a history of mental health illness and suicidal ideations but on admission, assessed to be not at risk of suicide. Multiple investigations by multiple departments were performed to determine the cause of the patient not able to close their right eye. Antibiotics were prescribed and therapy completed in hospital. On the 8th day after admission, discharge was planned. That same afternoon, the patient was found to have left the ward after receiving a phone call. Ward staff were not informed. 2 hours later, the police informed the hospital that the patient was found to have jumped from height. The case was reviewed and the overall care provided was appropriate.
