This alert describes six patients (2 male and 4 female patients, aged between 51 and 67) had committed suicide: two by jumping from height during home leave and two after found missing; one by suffocation with face towel; and one by electrocution. Each incident is described in the alert. Two recommendations for prevention of these fatal incidents are provided.

  1. Explore improvement of ward environment to serve the purpose of easy observation of high-risk patients with suicide tendency and require isolation for infection precaution.
