April 2016 - All of the medication safety actions scheduled for 2015-16 have begun and are progressing well towards completion. The actions continue to be well aligned and grounded in the identified goals of the action plan to centralize medication incident information to share learnings, implement error-prevention strategies, make opioid use safer for Canadians, improve communication at transitions in care, improve access to pharmacy in rural/remote areas, and improve input from patients and providers in medication safety technology development.
In addition to the successful launch of the 5 questions to ask about your medications list, completion is expected very soon of a white paper that identifies all of the separate reporting systems that collect medication incident data. Co-led by the Institute for Safe Medication Practices Canada and the Canadian Institute for Health Information, the white paper will provide valuable information to support developing a centralized access point where medication incident information from multiple sources can be pooled, synthesized and shared more effectively.
Other action leads Canada Health Infoway, Patients for Patient Safety Canada and the Canadian Centre for Substance Abuse continue to provide leadership in advancing actions in theme areas of technology and evidence informed practices.
