This safety notice highlights the potential medication incidents which could occur with new labelling of concentrated potassium chloride by Pfizer. The previous labelling of 10 mmol (0.75 g) in 10 mL Sterile Potassium Chloride Concentrate has been changed to 75 mg in 1 mL. Healthcare professionals typically prescribe and dispense potassium chloride using millimoles (mmol). While Pfizer will be revising the labelling to use the mmol designation, interim measures are required for this product. The notice provides details to be implemented to reduce the risk of medication errors.
