Lisa McCarthy, RPh PharmD MSc, Women's College Hospital
Lisa is a Pharmacist Clinician Scientist with Women's College Hospital and Women's College Research Institute and an Assistant Professor (status) with the University of Toronto. In partnership with ISMP-Canada, she is currently leading a CIHR-funded scoping review examining issues related to medication reconciliation in ambulatory care.
Purpose of the Call:
Women's College Hospital is an academic ambulatory hospital. The speaker will share their hospital’s journey as they sought to implement best practices for medication reconciliation from other settings customized for the ambulatory environment.
Specifically, she will:
Share findings from a recently conducted scoping review about medication reconciliation in ambulatory care settings .
Describe how medication histories from community pharmacists can be used in institutional ambulatory clinics (MedIntegrate program).
Highlight lessons learned when developing processes for medication reconciliation that are applicable to all health systems.
