This plenary provides PSEP – Canada Trainers with an overview of the patient safety movement and the shift to systems-based solutions for improving both safety and quality in healthcare. It summarizes progress to date, discusses challenges, and outlines a way forward. It welcomes all participants in healthcare to commit to being part of the solution.

the magnitude of the problem (large numbers of people are harmed);
the paradoxical role of progress in increasing complexity and risk; and
the challenges faced by provider communities in ensuring safety and transforming cultures.
