Although many healthcare organizations across Canada and internationally have found great value in the methods and tools described in the Canadian Root Cause Analysis Framework, much has been learned since the original publication in 2006
In 2009, the Canadian Patient Safety Institute (CPSI) began a comprehensive revision process including multiple consultations with users and experts from Canada and around the world. Some of the highlights of the revised Framework, now titled The Canadian Incident Analysis Framework include (click here for executive summary):
a big picture description of the incident continuum to better illustrate how incident analysis fits with all the other activities surrounding the management of patient safety incidents;
three tools for incident analysis (concise, comprehensive and multi-incident) to ensure effective use of the limited resources healthcare organizations dedicate to incident analysis;
a non-linear graphic that identifies contributing factors and the interconnections among them that reflects the complexity of the healthcare environment; and
guidelines for developing and managing recommended actions to help with prioritization to ensure those with the most impact are being implemented.
