Adverse event reporting and learning systems in healthcare have the potential to improve safety for all patients through the analysis of reported events, dissemination of recommendations for system improvements, and the local implementation of leading practices. This is achieved while maintaining a system-based emphasis on seeking and understanding the lessons that can be learned.
Foundational work has been assisted by Professor Lucian Leape and Dr. Susan Abookire to produce the World Alliance for Patient Safey’s WHO Draft Guidelines for
Adverse Event Reporting and Learning System (2005).
