We proudly recognize the excellent work Canadian hospitals have done in recent years to keep patients safe and improve the quality of care offered in our country. The High 5s initiative is intended to further support Canadian hospitals to continue their efforts in this regard. Based on the latest evidence, with support from experts in patient safety, an international team came together and developed standard operating protocols aimed to help hospitals achieve significant, sustained and measurable reduction in patient safety problems. A strong international learning network will support the project.
The three protocols address areas of significant concern for practitioners and of high risk for patients. The protocols can be easily integrated with current patient safety initiatives. They focus on correct site surgery, medication reconciliation, and managing concentrated injectable medicines.
Each protocol comes with comprehensive resources and strong project support; however it requires commitment from hospitals to ensure that the protocol is followed for the duration of the project. Roles and responsibilities of each partner are listed here.
