Saskatoon Health Region’s Quality Monitoring Framework on Long-Term Care, Quarterly Safety Report provides examples of performance measures for Long-Term Care.
The Board of Directors of the Sherbrooke Community Centre reviews quality improvement reports several times during the calendar year, including any sentinel event occurrences. Each reporting period focuses on specific quality indicators during a set time frame.
Accreditation Canada introduced six patient safety performance measures
into the Qmentum accreditation program. These indicators were selected based on a literature review and environmental scan, input from various stakeholders and alignment with other key initiatives in healthcare performance measurement. Indicators include:

Surgical Site Infection

, Health Quality Council of Alberta, report looks at quality measurement in Alberta’s healthcare system with a focus on acute care.
A Quality Improvement Action Form is prepared for the Board of Directors of the Sherbrooke Community Centre to review when the threshold for action for a standard/indicator is not met or when there is a significant change or improvement.

The BC Patient Safety & Quality Council’s
Measurement Strategies Report
provides a summary of successful measurement systems from around the world and how these systems have been used to improve the quality of healthcare. Information includes best practices for selection of indicators, displaying and analyzing data, and using data to drive improvement at different levels of an organization.
